Why adherence research matters in the Middle East
The Middle East carries one of the highest chronic disease burdens globally, with high prevalence of type 2 diabetes, hypertension, cardiovascular disease, and a growing incidence of cancer and autoimmune conditions. Yet treatment uptake and adherence rates in many of these conditions fall significantly below clinical guideline recommendations and below Western market benchmarks. The gap between epidemiological disease burden and treated patient population is commercially significant for pharmaceutical companies — it represents either market opportunity (if barriers to treatment initiation can be reduced) or commercial risk (if treated patients are discontinuing early and shrinking the revenue base).
Adherence research in the Middle East serves three strategic purposes. First, it quantifies the treated patient pool's real behaviour — how many patients prescribed a therapy are actually taking it as directed, and for how long? Second, it diagnoses the specific barriers driving non-adherence, enabling targeted intervention design. Adherence and persistence measure different dimensions of treatment-taking behaviour and require different measurement approaches. Understanding the distinction is essential for designing a Middle East adherence programme that produces the right evidence for the intended decision.
Adherence — also called compliance in older literature — measures the degree to which a patient takes their prescribed medication as directed: correct dose, correct timing, correct frequency. Adherence is typically expressed as a ratio: Medication Possession Ratio (MPR) measures the days of medication supply dispensed divided by the days in the observation period; Proportion of Days Covered (PDC) adjusts for overlapping refills and is considered the more conservative and preferred metric. Both can be calculated from pharmacy dispensing records if claims data is available, or estimated from self-report using validated instruments such as the Morisky Medication Adherence Scale-8 (MMAS-8).
Persistence measures the duration of continuous therapy before a patient stops treatment. It is typically expressed as the proportion of patients still on therapy at 90 days, 6 months, and 12 months from initiation, or as a median time to discontinuation. Persistence measurement requires a longitudinal data source — either pharmacy claims or a prospective patient follow-up study. In GCC markets, published 12-month persistence data for chronic disease treatments typically shows rates 15–25 percentage points below European benchmarks, driven by a combination of cost, side effect tolerance, and the health belief factors described below.

When patients transport these products from pharmacy to home, or during travel, brief temperature excursions above safe storage thresholds are common. In households without reliable refrigeration access — more prevalent in lower-income expatriate communities than in national populations — storage non-compliance may affect product efficacy. Cost sensitivity in non-Gulf Middle East markets. In Egypt, Jordan, Lebanon, and to a lesser extent in less-wealthy GCC communities, out-of-pocket drug costs drive adherence behaviour more directly than in high-income Gulf countries. For branded originators not covered by insurance, the cost barrier at refill is the primary driver of non-persistence. Research in these markets must capture the cost calculation patients make at each refill decision — whether the medication benefit justifies the out-of-pocket outlay — and where generic or cheaper alternatives are being substituted.
Patient education gaps and health belief barriers. Across Middle East markets, treatment discontinuation driven by misunderstanding of disease chronicity is a well-documented barrier. Patients with hypertension who feel well stop antihypertensive medication, believing the disease is resolved. Patients with rheumatoid arthritis stop biologic therapy during periods of low disease activity. Patients with type 2 diabetes stop oral agents when they lose weight and feel better. Social and family influences on treatment-taking. As with patient journey dynamics, adherence in GCC is influenced by family context. Disease areas with acute adherence challenges in Middle East
Certain therapeutic areas combine high disease burden with particularly challenging adherence profiles in Middle East markets, making them priority targets for adherence research investment.
Diabetes — oral and injectable. With type 2 diabetes prevalence among the highest globally, adherence to oral antidiabetic agents and insulin in the Middle East is a commercial and public health priority. Published research from GCC settings shows one-year PDC for metformin in the 55–65% range, substantially below the 75–80% threshold often used as a benchmark for adequate adherence. Cardiovascular disease — statins and antihypertensives. Statin adherence in Middle East markets is affected by the same drivers as in other regions — side effect concerns (myalgia, liver concerns), the "feeling well" discontinuation phenomenon, and cost — but at higher rates of discontinuation. One-year statin discontinuation rates in GCC cohort studies have been reported as high as 50–60%, versus approximately 30–35% in UK general practice data. Research methods for Middle East adherence studies
Middle East adherence research programmes use a combination of methods selected to match the data availability, timeline, and evidence objective.
Pharmacy claims analysis. Dispensing records from hospital pharmacy management systems (used by major MOH hospitals and the SAMED/Aramco system in KSA) and insurance claims databases (DAMAN in Abu Dhabi, DHA-linked insurance data in Dubai) provide objective, retrospective adherence data at scale. MPR and PDC can be calculated for large patient cohorts without the cost and time of prospective patient recruitment. The primary limitation is data access — institutional partnerships must be negotiated, and de-identification and data governance processes add pre-analysis setup time (typically 2–3 months). The payer perspective: how adherence data supports Middle East formulary negotiations
