    How the market is structured

    How ISP registration, Cenabast and FONASA versus isapres split demand

    ISP sanitary registration is the legal start, and for generics the agency expects bioequivalence against the reference product; foreign plants must show GMP that ISP will accept. After the registro, public and private paths diverge sharply. Cenabast runs competitive tenders for the national supply centre and for the Canasta Esencial de Medicamentos, then delivers into Servicios de Salud and municipal primary care; CIF Chile’s 2024 work showed Cenabast intermediation climbing from 61 percent of Servicio purchases in 2018 to 79 percent, so a missed lot is now a missed year of public volume. Law 21.198 also lets community pharmacies buy selected SKUs through Cenabast at capped resale prices, which has pulled some chronic packs out of unconstrained retail pricing. FONASA beneficiaries — more than four in five residents — receive those public packs at hospital and consultorio level, with GES guarantees covering a defined set of conditions. Isapre affiliates, a smaller and higher-income pool that Superintendencia timed at 2.63 million people in December 2024, still drive a disproportionate share of originator and private-clinic demand, even as the isapre financial crisis has pushed members toward FONASA. Retail is an oligopoly: Cruz Verde (and its Socofar wholesale arm), Farmacias Ahumada and Salcobrand negotiate listings that can make or break an OTC or chronic-repeat brand in Santiago, while independent farmacias hold more share in Valparaíso and the south.
